Hi.
These can be placed over a shower however i would recommend against it as the steam from the shower will shorten the life of the fitting.

It is possible to go in a wall however they wont work as well as the are designed to go into the ceiling.

Hi.
Yes you will need space in your ceiling cavity for installation.
I would say it may not work in your instance if there is no ceiling cavity.
